Tool #1: My Migraine Tips and Checklist

Who will transport your kids to school? Are there quick meals handy so you won't have to cook? Can someone at work cover for you while you're out?

Use this checklist to plan ahead—don't wait for your next migraine to get the answers. The next time a migraine hits, the checklist will help you manage your day-to-day routine.

Tool #2: Migraine Treatment Plan

It's becoming an accepted practice for patients with chronic conditions to have a record of their Treatment Plan, a document outlining any essential treatment information, including any medications.

Use your Treatment Plan to help family members, as well as people who are unfamiliar with your condition, understand what needs to be done when a migraine hits. It's especially useful when you're traveling far from home and will not have access to your personal physician.

If you don't yet have a treatment plan for your migraine, print out the template below and give it to your healthcare professional to fill in. Because you need to update the plan later, it's also a good idea to type your healthcare professional's instructions into the template and store it on your computer.
